Dermoscopy – the essentials

Summary
Dermoscopy (epiluminescece microscopy) is a dynamically developing diagnostic technique, which enables in vivo visualisation of epidermis and dermis structures. Dermoscopy links clinical dermatology and histopathological examination of surgically removed lesions. It is simple, non-invasive, and an easily repeatable method which significantly facilitates the differentiation between pigmented skin lesions. There are still a lot of difficulties in making proper diagnose of pigmented skin lesions, even for experts. Clinical appearance of melanoma and pigmented basal cell carcinoma often is very similar, and only histology decides about diagnose. Exact distinction between melanoma, basal cell carcinoma and benign melanocytic lesions sometimes allows to avoid unnecessary invasive interventions, like biopsy or excision and helps to find the best therapeutic methods.
It is also useful in diagnosis of inflammatory and infectious skin diseases, nail apparatus examination and hair diseases. The widest usefulness and the most important role of dermoscopy is in making differential diagnosis between benign and malignant melanocytic skin lesions (benign nevi, dysplastic nevi, melanoma). This technique increases the diagnostic accuracy of melanoma from 5% for 30%. Videodermoscopy brings a possibility of easy storage and retrieval of lesions during follow-up examinations. It also allows to create a so called "body-maps” and to analyze new skin lesions. Dermoscopy not only increases the diagnostic accuracy of skin cancer or melanoma, but also facilitates the proper diagnosis of benign skin lesions.
